In-Home Physical Therapy Session:

Description

This service is available for Manhattan-NYC residents, and it takes place at the client’s home.

The first session  (pulmonary, cardiac, or scoliosis conservative management evaluation) consists of:
• Interview with the client on their past and current medical and social circumstances.
• Assessing gross range of motion, muscle strength, and endurance.
• For Scoliosis clients only: reviewing X-Rays of spine and discussing conservative management of Scoliosis with the Schroth method.
• Formulating an exercise plan for the follow-up sessions.
• Minimal exercise based on the full assessment.

The follow-up sessions consist of:
• Proper performance of the program.
• Warm-up exercises (stretching and breathing).
• Aerobic exercises (with clients’ equipment, or without).
• Strengthening exercises for arms and legs (with clients’ equipment or without).
• Functional exercises (sit to stand and step up).
• Cool down exercises (stretching, breathing, and relaxation).
• Monitoring vitals and symptoms.
• Exercise progression.
• Research-backed education.

The program incorporates a variety of movement and breathing techniques from Physical Therapy, Pilates, Yoga, Gyrokinesis, Tai-chi, Qi-Gong, Schroth Best Practice Method, and the Buteyko and Bradcliff Breathing Methods.


Prerequisites:
In order to participate in this program, the client and/or client’s caregiver is required to:

• Have a good insight into their medical condition.
• Follow up regularly with their medical doctors.
• Be compliant with their medical regimen.
• Be hemodynamically stable (stable blood pressure and heart rate).
• Own a glucometer if diabetic and able to check their glucose.
• Be able to follow directions.
• Be able to walk and/or transfer independently or with one person's assistance.
• Have clearance and referral from their medical doctor in order to start an exercise program, especially if recently discharged from a hospital.
• For Scoliosis clients only: regular followup with an orthopedic doctor regarding your scoliosis and referral for conservative management of scoliosis with the Schroth method.

It is recommended that you complete at least 8 weeks of exercise (two sessions/week). Research shows that this is the minimum length of cardiopulmonary exercise program needed to notice improvements in your health. Each session is 60 minutes. The physical therapist will assess and determine the beneficial program length for the client. Additionally, the client’s availability and preferences are taken into consideration when determining the program length and frequency.

FAQ

Can I participate in the program if I don’t reside in Manhattan, NY?
No. The in-person physical therapy services are offered only for residents in Manhattan at this time. However, keep checking our website for soon-to-launch Virtual Physical Therapy where you may participate in this program from anywhere in the world.

Why choose in-home physical therapy and rehabilitation program?
Research shows that participating in an in-home and/or tele-cardiopulmonary exercise program of at least 8 weeks in length has a positive impact on an individual’s health. This program is under the professional guidance of a licensed Physical Therapist with extensive experience in the field of cardio-pulmonary rehabilitation.
-You will exercise in the comfort of your own home.
-You will receive a personalized treatment session tailored to your needs with 1:1 supervision.
-You will have your vitals and symptoms monitored during the exercise session.

Who can participate in this program?
This exercise program is suitable for clients diagnosed with cardiac (EF equal or greater than 30%) or pulmonary medical conditions (chronic and/or post-surgical). The clients must follow up with their medical doctor regularly, be compliant with their medical regimen, and are hemodynamically stable (stable blood pressure and heart rate).

Can I participate in the program if I’m diagnosed with heart failure and/or have an LVAD device?
At this time, only clients diagnosed with heart failure with an Ejection Fraction (EF) of 30% and > may participate in this exercise program. You will need to provide proof in a written format of your most recent echocardiogram that shows your EF. This program is not suitable for clients with LVAD devices.

Can I participate in the program if I have episodes of chest pain at rest randomly or for no apparent reason?
No. Chest pain at rest for no apparent reason is called ‘unstable angina’ and requires immediate medical examination.

Can I participate in the program if I use supplemental oxygen?
Yes. The physical therapist will monitor your vitals during exercise and titrate your supplemental oxygen as necessary. You will be required to have an oxygen concentrator at home (stationary and/or portable) that will be available for use during the exercise session.

How many physical therapy sessions do I need? How long is one session? And what is the frequency?
Research shows that at least 8 weeks (two sessions/week) of cardiac or pulmonary rehabilitation is needed to notice improvements in your health. One session is 60 minutes. The frequency is generally 2 times per week. The physical therapist will determine the client’s beneficial program length. And the client can choose the session frequency that is suitable for them.

Do I need a referral from my medical doctor in order to participate in this program?
Yes. To participate in this exercise program, you will need a referral from your medical doctor in a written format.
Please note that referrals for cardiac rehabilitation must be ordered by the clients’ Attending Cardiologist (referrals from PCP, PA, or NP will not be accepted).

Can I participate in the program if I don’t have exercise equipment at home?
Yes. Although equipment such as a treadmill or a stationary bicycle is recommended for the aerobic component of this program, other aerobic exercises can be performed without equipment as well.

Can I participate in the program if I require 2-person assistance for mobility?
No. For your safety, only clients who can walk and/or transfer independently or require one-person assistance may participate in this exercise program.

Who is not eligible to participate in the  Schroth method for conservative management of scoliosis?
Clients diagnosed with the following medical conditions are not eligible to participate in the Schroth method for conservative management of scoliosis:
-Moderate to severe osteoporosis
-Marfan’s syndrome
-Circulatory disorders
-Tuberculosis of lungs and bones
-Spastic paralysis
-Dementia and other cognitive disorders
-Lobe or lung resection

What happens after I book a consultation?
You will receive a confirmation email from Move and Breathe With Me. The email will request for more details such as address, a referral for exercise, and other medical information as necessary.

Do you accept health insurance?
Not at this time.

What is your cancellation policy?
If you cancel at least 24 hours before your scheduled appointment, you will get a full refund. If you cancel less than 24 hours before your scheduled appointment, there will be a cancellation fee of $50, unless there is a medical circumstance and you can show proof (in this case, you will receive a full refund). All cancellations and rebooking on services must be done so via our website or email.
